DOMS机制和治疗手段的最新研究进展

摘 要: 离心运动导致了延迟性肌肉酸痛(delayed onset muscle soreness,DOMS)的发生,其准确的机制及治疗措施至今仍未确定,但普遍认为炎症、自由基导致了肌肉酸痛,可使用抗炎性药物、抗氧化物补剂治疗。然而,近来的研究表明:延迟性肌肉酸痛是多因素造成的结果,包括炎症、神经释放因子、瞬变受体电位通道等,且两条途径介导了延迟性肌肉酸痛发生,一条是通过Ⅲ—Ⅳ类传入神经伤害性受体介导的疼痛发生,即外周机制;另一条是通过Ⅰ—Ⅱ类传入神经机械受体介导的疼痛发生,即中枢机制。研究趋势表明:运动性炎症、活性氧有益于骨骼肌适应。这意味着延迟性肌肉酸痛的治疗措施将发生变化。因此,综述了延迟性肌肉酸痛机制的最新研究进展及其相应的治疗措施。

Abstract: It has been well known that eccentric exercise leads to delayed onset muscle soreness (DOMS). The exact mechanism and treatment strategies remain uncertain, but it has been generally accepted that inflammation and free radical induced DOMS, and nonsteroidal anti-inflammatory drugs and antioxidants were used for the treatment. However, recent study suggests that DOMS is induced by many factors, and two ways mediate DOMS, where in one way mediates pain via nociceptors served by GpⅢ—GpⅣ afferent fibers, which is peripheral mechanism, and the other way mediates pain via mechanoreceptors served by GpⅠ—GpⅡafferent fibers, which is central mechanism. Researches indicate that exercise-induced inflammation and reactive oxygen species are of benefit to adaptive effects of skeletal muscle. This means that treatment strategies of DOMS will be changed. Therefore, the paper summarizes recent advances in the research of the mechanism of DOMS and corresponding treatment strategies.
