层次分析法在社区医务人员综合评价心脑血管病防控措施中的应用

摘 要: 目的:为达到慢性非传染性疾病(简称慢性病)社区预防与控制(简称防控)工作高效率运行目的,本文通过综合措施权重的研究介绍一种有效的综合评估方法与技术。方法:用两两比较的方法在社区医务人员中开展调查。应用层次分析法(analytic hierarchy process,AHP),在建立矩阵模型的基础上对调查信息进行特征向量计算和一致性检验分析,最终通过算术平均数的计算获得各项措施的权重比例。结果:建立了措施选择、准则和方案3层11项元素的1个结构模型,据此形成了6个矩阵模型,内容覆盖国家防治慢性病规划中5项评价要素和苏州市现有28个工作项目。本次共调查36人,平均年龄38.69岁,参与慢性病防控工作时间平均为7.28年。调查对象调查信息准则层、方案层中的减少死亡、减少发病、降低疾病负担、控制危险因素和提高疾病管理量6项符合一致性要求的人数分别占调查人数的30.56%、50.00%、50.00%、66.67%、61.11%和61.11%. 以符合一致性要求调查信息为数据源所获得的权重比例为健康教育25.04%、全民健康生活方式行动支持性环境建设24.05%、心脑血管病管理17.51%、控烟17.37%和社区高危人群筛查与管理16.02%. 结论:基层社区医务人员认为健康教育和全民健康生活方式在慢性病防控工作中地位最为重要,研究结果给出了明晰的数量值。研究结果有助检测现有工作的不足和指导调整方案的策划。

Title: Application in analytic hierarchy process in comprehensive evaluation for CCVD prevention and control measure from GP

Abstract: Objective: To achieve high performance for chronic non-infectious disease prevention and control in community, an effective method and technology for comprehensive evaluation was introduced with the study on comprehensive measures’ weight. Methods: Firstly, the questionnaire was conducted from the general practitioners (GP) for pairwise comparison. Then, the eigenvector was calculated and data consistency was tested on the basis of building matrix models through the application of analytic hierarchy process (AHP). Finally, various measures’ weight was obtained through the calculation of arithmetic average. Results: A structure model which contained 3-levels (measures, criteria and projects) and 11 elements was successfully built. Then, 6 matrix models were generated including 5 key elements of national planning for chronic diseases prevention and control, 28 current work items of Suzhou according to the structure model above. The average age and average work age of 36 participators in our investigation were 38.69 years old and 7.28 years, respectively. The proportions of participators which fit the relative consistencies in criteria level and project level such as reducing death, disease occurrence, disease burden, control risk factors and increasing quantity of disease management were 30.56%, 50.00%, 50.00%, 66.67%, 61.11% and 61.11%, respectively. Furthermore, the weights of health education (25.04%), supportive environment construction of national action of healthy life style (24.05%), cardio-cerebrovascular disease (CCVD) management (17.51%), smoking control (17.37%), screening and managing high-risk population of CCVD in community (16.02%) were obtained from the above data source. Conclusion: The GP thought that health education and national action of health life style were the most important in all measures, and a clear number of values were provided in the study. With these results, we can find out the deficiency of current work and get guidance for adjusting work projects.
