二甲双胍治疗Ⅱ型糖尿病作用机制的代谢组学研究

摘 要: 目的:研究Ⅱ型糖尿病(DM-Ⅱ)患者及给与降糖药二甲双胍治疗后的患者血清代谢物谱变化,从代谢组学的角度阐述二甲双胍治疗Ⅱ型糖尿病的作用机制。方法:采用超高效液相色谱串联质谱(ultra performance liquid chromatography tandem mass spectrometry,UPLC/MS/MS)技术对20名糖尿病患者及15名给与二甲双胍治疗后的患者血清代谢物谱进行分析测定。采用主成分分析(principal components analysis,PCA)和偏最小二乘-判别分析(partial least squares-discriminant analysis,PLS-DA)模型对给与二甲双胍治疗后和未经治疗的Ⅱ型糖尿病患者血清代谢物谱进行分类。结果:二甲双胍治疗后的患者与未经治疗的Ⅱ型糖尿病患者相比血清代谢物谱发生了明显的改变。发现了5个潜在的生物标记物,对药物可能的作用机制进行了讨论。结论:二甲双胍可以调节Ⅱ型糖尿病患者体内代谢紊乱,其作用机制可能与降低血清溶血磷脂酰胆碱有关。

Title: Metabonomic study on the action mechanism of metformin in the therapy of type Ⅱ diabetes mellitus

Abstract: Objective: To study the metabolic differences between type Ⅱ diabetes mellitus patients and patients treated with metformin and elucidate the action mechanism of metformin from the angle of metabonomic. Methods: Ultra-performance liquid chromatography tandem mass spectrometry (UPLC/MS/MS) was applied to the serum metabolite profiling in type Ⅱ diabetes mellitus (DM-Ⅱ) patients and patients treated with metformin. Principal components analysis (PCA) and partial least squares-discriminant analysis (PLS-DA) models were employed for the discrimination of the two groups. Results: Significant difference in endogenous metabolite profiles was observed in DM-Ⅱ patients and metformin treated patients. Five potential biomarkers were discovered. Possible action mechanism of metformin was discussed. Conclusion: Metformin can regulate the metabolic disorders in DM-Ⅱ patients, which may be associated with its effect to lower serum lysophosphatidylcholines.
