心率变异性在动态劳动负荷评价中的应用

摘 要: 目的:探讨用心率变异性(heart rate variability,HRV)评价动态劳动负荷的可行性。方法:46名受试者在二阶梯踏台(58 cm×35 cm×23 cm)上以不同速率进行上下台阶运动,模拟动态劳动负荷。采用Holter 8800型动态心电图分析仪连续记录其心电信号,并将心电信号转变为数字信号,应用Holter分析软件分析心率变异性的频域指标:总功率(total power,TP)、低频(low frequency,LF)、高频(high frequency,HF)和LF/HF.同时记录和分析受试者运动过程中的肺通气量、心率、主观疲劳感的变化。结果:1) 肺通气量、心率和Borg量表分值在3种速率下的差别均有显著性(P<0.05),并且随着负荷的增加而升高,呈明显的剂量-反应关系。2) LF 和HF在动态运动中均低于休息时的值(P<0.05),随着劳动负荷量的增加,LF升高而HF却降低;TP和LF/HF在动态运动中均高于休息时的值,并且随着负荷量的增加而升高。结论:动态劳动负荷可采用传统的指标,如肺通气量、心率进行评价,也可以采用Borg量表和心率变异性进行评价。

Title: Application of heart rate variability in evaluation of dynamic workload

Abstract: Objective: To investigate whether heart rate variability (HRV) can be used to evaluate dynamic work1oad. Methods: 46 volunteers went up and down on a two-ladder platform (58 cm×35 cm×23 cm) at different speeds to simulate dynamic workload. ECG signals were recorded by Holter 8800 during the performance and transferred into digital signals. Total power (TP), low frequency (LF), high frequency (HF) and the ratio of HF to LF were analyzed by Holter software. At the same time, pulmonary ventilation volume, heart rate and values of Borg scale were record and analyzed. Results: 1) the pulmonary ventilation volume, the heart rate and the values of Borg scales at the three speeds all had significant differences (P<0.05), and all increased while the speed increased, showing a significant dose-response relationship. 2) All the values of HF and LF in the three dynamic exercises were lower than those in rest time (P<0.05). While the speed increased, the values of LF increased, however, the values of HF decreased. All the values of TP and the ratio of HF to LF in the three dynamic exercises were higher than those in rest time. While the speeds increased, the values also increased. Conclusion: The dynamic workload can be evaluated by Borg scale and heart rate variability as well as conventional indexes including pulmonary ventilation volume and heart rate.
