应用脂肪干细胞复合膀胱去细胞基质修复SD大鼠膀胱缺损研究

摘 要: 目的:以膀胱去细胞基质(bladder extracellular matrix,BECM)为组织工程支架材料,复合脂肪干细胞(adipose-derived stem cells,ADSCs)后通过手术原位植入预先切除正常膀胱的Sprague Dawley大鼠(SD大鼠),观察干细胞在大鼠体内对膀胱组织再生和功能重建的效应。方法:40只平均体重为250 g的SD大鼠随机分为实验组和对照组,均手术切除正常膀胱,仅保留膀胱三角区。酶消化法获得大鼠脂肪干细胞,种植于膀胱去细胞基质中,用于修复实验组SD大鼠膀胱,对照组仅用膀胱去细胞基质修复。术后1,2,4,8周进行膀胱尿流动力学测定并以免疫组化法评价2组间新生血管、平滑肌,以及神经再生的情况。结果:实验组和对照组术后1周膀胱形态功能无明显差别,自术后2周开始实验组新生血管、平滑肌,以及神经再生情况均优于对照组。膀胱容量及膀胱最大压在修复后1周无明显差异,实验组术后2周测得膀胱最大压,至8周可见,实验组膀胱容量较大且膀胱顺应性增高。结论:SD大鼠脂肪干细胞与膀胱去细胞基质结合良好,脂肪干细胞-膀胱去细胞基质复合材料在实验中可促进血管、神经、平滑肌等结构的再生,并对修复后膀胱的功能重建发挥作用。

Title: Study on grafts of bladder extracellular matrix seeded with adipose derived stem cells for reconstitution of bladder defect of SD rats

Abstract: Objective: In situ grafting bladder extracellular matrix (BECM) as tissue engineering support material seeded with adipose-derived stem cells (ADSCs) to SD rats resection of bladders for investigating the effect of tissue engineering with ADSCs on regeneration of the bladder acellular matrix grafts and reconstitution of bladder defect. Methods: 40 female SD rats weighing 250 g were divided into two groups after resection of all the bladders, only trigones left. In the experimental group, BECM seeded with ADSCs was grafted to the host bladder. In the control rats, augmentation was used with unseeded BECM. At 1, 2, 4, 8 weeks after grafting, cystometry was performed. Immunohistochemical method was used to assess neovascularization, smooth muscle and nerve regeneration. Results: There was no significant difference in functional and histologic evaluations between the two groups 1 week after grafting. The contents of smooth muscles, blood vessels and nerves were significantly greater in the seeded group than in the control group 2 weeks after grafting. The capacity and maximum bladder pressure the had no significant difference within 1 week after grafting, the maximum bladder pressure of the experimental group was obtained 2 weeks after grafting, and the capacity and compliance of ladder were increased in the seeded group compared to the control group after 8 weeks. Conclusion: The BECM has good compatibility with adipose-derived stem cell. The ADSCs seeded in BECM can promote angiogenesis, neurogenesis, and muscular regeneration and has positive effect on bladder functional reconstitution.
