原因不明复发性自然流产蜕膜TLR4对调节性T细胞免疫抑制功能的影响

摘 要: 目的:探讨原因不明复发性自然流产(unexplained recurrent spontaneous abortion,URSA)患者蜕膜中Toll样受体(Toll-like receptor,TLR)对调节性T细胞(regulatory T cells, Treg)免疫抑制功能的影响。方法:采用RT-PCR法定量分析TLR mRNA在人蜕膜T细胞中的表达种类;流式细胞术(flow cytometry, FCM)法分析TLR4在蜕膜Treg 细胞上的表达频率;体外用TLR特异性配体刺激分离出的Treg细胞,观察TLR4对Treg细胞增殖、免疫抑制功能的影响,以及在此作用通路下Treg细胞对自然杀伤(natural killer, NK)细胞功能的调控。结果:URSA组蜕膜Treg细胞上TLR4表达显著高于对照组(P=0.004)。TLR4特异性配体脂多糖(lipopolysaccharide,LPS)可促进体外纯化Treg细胞增殖,增强Treg细胞对效应T细胞增殖的抑制和对NK细胞的毒性抑制。但当Treg细胞与去除T细胞和NK细胞的蜕膜淋巴细胞悬液共培养时,LPS却降低Treg细胞对效应T细胞和NK细胞的抑制功能。结论:URSA中可能存在TLR4信号途经的异常激活,致使Treg细胞的免疫抑制功能降低,以及NK细胞的异常活化和毒性增加,最终导致母胎界面免疫耐受状态失衡。

Title: Effect of decidual TLR4 on regulatory T cell function in patients with unexplained recurrent spontaneous abortion

Abstract: Objective: To investigate the expression of Toll-like receptor (TLR) on T cells in deciduas and their effect on immunosuppressive capacity of regulatory T cells (Treg) in patients with unexplained recurrent spontaneous abortion (URSA). Methods: The expression of TLR mRNA on Trge cells was examined by RT-PCR. The proportion of TLR4 in Trge cells was detected by flow cytometry (FCM). The effects of TLR4 on Treg cell proliferation, immune suppression, and the regulation of Treg cells to natural killer (NK) cell function were observed using the isolated Treg cells by TLR specific ligand stimulation in vitro. Results: Compared with control group, higher expression of TLR4 was detected on Trge cells in deciduas (P=0.004). LPS could enhance the immunosuppressive capacity of Trge cells, and also could facilitate the proliferation of Trge cells. Engagement with LPS, purified Trge cells could inhibit the function of NK cells in vitro without the presence of other immunological cells. However, the immunosuppressive capacity of Trge cells was decreased with the existence of other immunological cells. Conclusion: There may be an abnormal activation of TLR4 pathway in URSA, which led to the decrease of immunosuppressive capacity of T cells and the abnormal activation and toxicity increase of NK cell, and therefore, this led to imbalance of maternal fetal interface state of immunological tolerance.
