采用Bland-Altman分析评价PC MR法和MR心脏容积法所测每搏输出量的一致性

摘 要: 目的:探讨Bland-Altman分析在评价相位对比磁共振(phase contrast magnetic resonance,PC MR)法和MR心脏容积法测量每搏输出量(stroke volume,SV)一致性的应用。方法:对22例正常志愿者分别采用PC MR法和MR心脏容积法测量SV,并采用配对t检验、相关分析和Bland-Altman 3种统计分析方法对所测结果进行一致性评价。结果:经配对t检验,PC MR法和MR心脏容积法所测的SV具有统计学差异(P<0.05);相关分析显示,PC MR法和MR心脏容积法测量SV具有良好的相关性(R =0.95)。Bland-Altman分析表明:PC MR法测量的SV低于MR心脏容积法测量的结果,100%的点在一致性界限范围内;PC MR法和MR心脏容积法测量SV差值的最大绝对值为9.48 mL,小于生理性SV的波动范围及临床允许的测量误差值。结论:相比配对t检验、相关分析,Bland-Altman分析能给出更合理的结果,是一种理想的一致性评价方法。

Abstract: Objective: To investigate the application of Bland-Altman analysis for evaluation of consistency of stroke volume (SV) which were measured by phase contrast magnetic resonance (PC MR) and cardiac MR. Methods: SV was determined by measuring the blood flow in the beginning of aorta and which by measuring the change of left ventricular of 22 healthy volunteers within each cardiac cycle. The clinical consistency of PC MR and cardiac MR was respectively evaluated by paired-samples t test, correlation and Bland-Altman analysis. Results: The paired-samples t test showed that the SV measured by PC MR and cardiac MR had statistical differences (P<0.05). The correlation showed there was good relationship between the values of SV measured by PC MR and cardiac MR (R=0.95). However, Bland-Altman analysis revealed that the values of SV measured by PC MR were less than cardiac MR. 100% of the points were in the limits of agreement, the maximum absolute value of difference (9.48 mL) was in the acceptable range of physiological fluctuations of SV and clinical admissible error. Conclusion: Compared with the paired-samples t test and correlation, Bland-Altman analysis can give more satisfactory result, so it is a perfect method for evaluation of consistency.
