健康老年人肠道定植产ESBLs大肠埃希菌的ISCR增高细菌耐药性的研究

摘 要: 目的:调查至华山医院体检的健康老年人肠道定植产超广谱β-内酰胺酶(extended-spectrum β-lactamases,ESBLs)大肠埃希菌中常见插入序列共同区(insertion sequence common region,ISCR)阳性率及ESBLs的基因型。方法:从华山医院体检健康老年人的肠道分离大肠埃希菌,采用琼脂稀释法进行药敏试验,检测菌株耐药性情况。应用PCR方法,确认ISCR阳性率及ESBLs的基因型。依据是否含ISCR序列,将上述菌株分为两组,比较两组细菌对各种抗生素的耐药率。结果:在262株肠杆菌科细菌中,基因阳性率分别为ISCR1(22.9%)、ISCR2(10.3%)、ISCR3(5.3%)、ISCR4(5.3%)、ISCR5(1.9%)、CTX-M-1(34.3%)、CTX-M-9(69.5%)、CTX-M-2(2.3%)、SHV(0.4%)、TEM(42.0%). 各种抗生素的耐药率分别为阿米卡星(0.8%)、头孢噻肟(99.6%)、磷霉素(3.4%)、头孢吡肟(13.0%)、头孢他啶(22.9%)、头孢西丁(9.9%)、左氧氟沙星(46.6%)、氨苄西林(100%)、氨苄西林舒巴坦(57.6%)、哌拉西林他唑巴坦(0)、美罗培南(0)、厄他培南(0)。除美罗培南、厄他培南及哌拉西林他唑巴坦这三种耐药率为0的抗生素外,含ISCR菌株对大多数抗生素的耐药率均比不含ISCR菌株高(阿米卡星除外)。结论:ISCR可能在产ESBLs大肠埃希菌耐药性增高中发挥重要作用。

Title: ISCR increases bacterial resistance in ESBLS-producing Escherichia coli isolated from healthy elderly peopel

Abstract: Objective: To investigate the prevalence of insertion sequence common region (ISCR) and the genotype of extended-spectrum β-lactamases (ESBLs) among the ESBLs-producing Escherichia coli isolated from healthy elderly of Huashan Hospital. Methods: PCR was used to detect the gene of ISCR1, ISCR2, ISCR3, ISCR4, ISCR5, CTX-M-1, CTX-M-9, SHV, TEM and CTX-M-2 among the E. coli isolated from healthy elderly people of Huashan Hospital. Antimicrobial susceptibility test was performed by agar dilution method. These strains were divided into ISCR-positive and ISCR-negative groups. The antibiotic resistance rates of the two groups were compared. Results: In 262 strains, the positive rates of the genes were ISCR1 (22.9%), ISCR2 (10.3%), ISCR3 (5.3%), ISCR4 (5.3%), ISCR5 (1.9%), CTX-M-1 (34.3%), CTX-M-9 (69.5%), CTX-M-2 (2.3%), SHV (0.4%), TEM (42.0%). Each antibiotic resistance rate was listed as follows: Amikacin (0.8%), Cefotaxime (99.6%), Fosfomycin (3.4%), Cefepime (13.0%), Ceftazidime (22.9%), Cefoxitin (9.9%), Levofloxacin (46.6%), Ampicillin (100%), Ampicillin-Sulbactam (57.6%), Piperacillin-Tazobactam (0), Meropenem (0) and Ertapenem (0). The group containing ISCR had higher resistance rate in most antibiotics, except for Amikacin and other three antibiotics with the resistance rate of zero (Meropenem, Ertapenem, and Piperacillin-Tazobactam). Conclusion: ISCR may be associated with higher antibiotic resistance in ESBLs-producing E. coli.
